COMPANY INFORMATION
TEXAS NOTICE TO CLIENTS: ACCESS TO RECORDS & COMPLAINTS
In accordance with Texas Health and Safety Code § 181.105, Fuego Therapy Counseling Services PLLC provides the following instructions regarding your health records and your rights as a client.
1. HOW TO REQUEST YOUR HEALTH CARE RECORDS
You have the right to access and receive copies of your health care records. To ensure the security of your Protected Health Information (PHI), Fuego Therapy Counseling Services PLLC requires that all record requests be submitted through our secure HIPAA-compliant portal.
Instructions: Please log in to your Client Portal through SimplePractice https://fuegotherapycounselingservices.clientsecure.me/sign-in. Navigate to the messaging or documents section to submit your written request for records. Otherwise, you can also submit a request in writing at the office if attending an in person session. Records request form can be attained from the Front Desk Administrator.
Timeline: In accordance with Texas law, we will respond to your request within 15 business days.
Fees: We may charge a reasonable, cost-based fee for the retrieval and copying of records as permitted by Texas Law.
FUEGO THERAPY COUNSELING SERVICES PLLC TEXT MESSAGING TERMS OF USE
By “Opting In” to or using a “Text Message Service” (as defined below) from FUEGO THERAPY COUNSELING SERVICES PLLC, you accept these Terms & Conditions.
This agreement is between you and FUEGO THERAPY COUNSELING SERVICES PLLC or one of its affiliates. All references to “FUEGO THERAPY COUNSELING SERVICES PLLC,” “we,” “our,” or “us” refer to FUEGO THERAPY COUNSELING SERVICES PLLC, 201 Billings St. Suite 510, Arlington, Texas 76010.
CONSENTING TO TEXT MESSAGING
By consenting to receive text messages from us, you agreed to these Text Messaging Terms and Conditions, as well as our Privacy Policy & Terms and Conditions https://www.fuegotherapy.com/privacy-policy/ incorporated herein by reference.
PROTECTED HEALTH INFORMATION (PHI)
We are committed to protecting your privacy and complying with the Health Insurance Portability and Accountability Act (HIPAA). Please note that we will not send Protected Health Information (PHI) via text message. Our text messaging service is intended for general communication only, such as appointment reminders, scheduling, or administrative matters.
